The contracted Hospitalists are responsible for the following:

  • All urgent and emergent admissions.
  • Provision of internal medicine, pulmonary disease and critical care medical services to all hospitalized adult patients.
  • Coordination of medical care and inpatient utilization management for all surgical patients.
  • Consultation on complicated surgical and obstetric cases .
  • Coordination of all ancillary services related to the inpatient episode of care, inclusive of durable medical equipment, home health and infusion services, etc..
  • Coordination, in conjunction with the discharge planner, of discharge planning needs with the patient and the patient's family.
  • Feedback to the Primary Care Physician, as required, inclusive of a discharge summary within 24 hours of discharge.
  • Coordination of transfer for out-of-area patients to an in-plan hospital.




The Primary Care Physicians are responsible for the following:

  • When a patient requires a non-scheduled admission, please immediately contact the appropriate hospitalists delineated above by Hospital and age group. These providers can be reached at the numbers indicated.
  • When a Global Care Medical Group IPA patient presents to the Emergency Room, the patient's PCP should be contacted for authorization. If it is appropriate to admit the patient following the ER screening, the hospitalist should be contacted for admitting orders.
  • When available, medical history should be provided to ensure continuity of care.
  • Although the hospitalist will be admitting and caring for the Global Care Medical Group patient while they are in the hospital, the PCP remibursement rates will remain the same. It will remain the responsibility of the Primary Care Physician's office to arrange for hospital bed availability should the admission occur during regularly scheduled office hours. We appreciate your support of the Hospitalist Model, and stress the importance of following the protocols set forth above with regard to all inpatient admissions and hospital stays.
